Contract from the Agency for Healthcare Research and Quality (AHRQ) to improve the delivery and efficiency of chronic care through the spread of the Chronic Care Model to numerous primary care practices that serve vulnerable populations, document resulting quality improvement activities and their impact, and to gain consensus in the filed about the evidence regarding practice coaching and creating a research agenda on practice coaching. Held two-day consensus panel at the Cathedral of our Lady of the Angels in January 2010. Currently recruiting practices for the practice coaching intervention to begin in August 2010.
