Americans in Motion - Healthy Interventions
The AAFP is seeking to position fitness as “the treatment of choice” when dealing with issues of prevention and treatment of chronic conditions.
The AIM-HI study will bring together the American Academy of Family Physicians, 8 federally-funded Centers for Obesity Research (COR), the editors of Obesity magazine, and two primary care practice-based research networks (LA Net and PRIME-NET) to implement and evaluate culturally appropriate interventions to increase “fitness” (physical activity, nutrition and emotional well-being) among low-income, minority patients.
Ultimately, this program will seek to improve care for all patients through fitness-related physician interventions.
Goals of project
- To implement a culturally appropriate fitness intervention in 6 primary care practices in Los Angeles
- To evaluate the reach and effectiveness of the intervention with low-income, minority patients
The Intervention: a two-level randomized controlled trial
The first level randomization will be performed at the practice level. This will allow the research team to study the impact of involving both clinicians and office staff working on personal changes (i.e., creating the “healthy” office) – a central theme of AIM – on the translation of the project’s educational tools to patients.
The second level of randomization will occur at the patient level to evaluate the effectiveness of feedback of two novel physiologic measures, Homeostatic Assay – Insulin Resistance (HOMA-IR) and Nuclear Molecular Resonance lipoprotein profiles (NMRLP) on self-reported diet, physical activity and emotional well-being and established physiologic measures of behavioral change.
The effectiveness of the intervention on patient-level behavioral change and additional physiologic measures of increased exercise and improved diet will be evaluated for all patients by baseline, 6-month, and 12-month measures.

AIM-HI Team Member Update: January 22, 2008
LA Net clinics have had a fantastic start to AIM-HI and are doing a great job both rolling out the study with all adult patients and enrolling research study patients! We are excited to welcome Dr. Wilson Pace to the traditional LA Net clinics (Highland Park, Bell Gardens and Hawaiian Gardens) next week! Below is a copy of the schedule for those days. I have a few housekeeping tips and some questions I need responded to by each clinic.
Housekeeping:
- For any clinic that has a staff member who would like to participate as a research study patient, please contact Angie Meers at the AAFP (ameers@aafpr.org or 913-906-6000) before you enroll them into the study.
- Please remember to fax your weekly AIM-HI visit reminders to the AAFP. Let me know asap if you do not have this form. This should be faxed every week, regardless of if you see new patients, to keep the AAFP up to date on where each clinic is and how funds are dispersed to each clinic.
- Traditional clinics (Hawaiian Gardens, Bell Gardens and Highland Park) we still need more patient exit surveys and clinician health surveys. Please contact me if you have questions or need assistance with sending surveys to the AAFP.
- Enhanced clinics (TCC Main and TCC Pine) we have had trouble contacting the staff members nominated for your staff posters - please send me their contact info (email and phone).

AIM-HI Team Member Update: January 2, 2008
Happy New Year! 2008 is here and that means the launch of AIM-HI with patients!
This week you should begin implementing AIM-HI with ALL adult patients. Next week (Jan 7) we will begin enrolling patients for the research study.
Please take some time to review the AIM-HI tools with staff at your clinic, make sure your posters and flyers are visible, and make sure everyone has their AIM-HI buttons on!

AIM-HI Team Member Update: December 4, 2007
Everyone has been doing an excellent job so far! We're all very excited to begin AIM-HI roll out with patients in January! Here are some updates and housekeeping items to make sure roll out runs smoothly - Traditional Clinics, please note a special notice at the end of the email to you.
Surveys:
- Currently you should be working on the Clinician Health Survey (going from Dec 3- Dec 7)
- Next week (Dec 10 - Dec 14) is the Patient Exit Survey. You should have a packet of surveys and a survey collection box sent directly to your clinic. If you haven't received this by Thursday of this week (Dec 6) let Laura know.
Human Subjects Certification:
- Please have all staff members significantly involved in the study, including physicians who will be inviting patients into the study, complete the Human Subjects Certification.
- You can complete the certification online at: http://www.citiprogram.org/
- Please send Laura a pdf of your final certification.
- This should be completed by the end of December and is very important! This must be completed before working with patients on AIM-HI.
Needles:
- There are two types of needles available for the blood draws of the 30 study patients. Please let me know which of the following you would prefer to have sent to your clinic: BD Vacutainer Safety-Lok Blood Collection Set 25G x 3/4 x 12 (these are the 'butterfly' sets)
OR BD Vacutainer Eclipse Blood Collection Needles 22G x 1-1/4

Patient Enrollment and Study Rollout:

AIM-HI Team Member Update: November 8, 2007
I'd like to congratulate everyone on the great job you're doing so far! We've been getting excellent feedback at the national level for the creative ideas and dedication of the Southern California clinics! Below is information on upcoming surveys and some ideas about where you should be in terms of your clinic's action plan.
Upcoming Surveys:
- Clinician/Staff Health Survey - December 3-7
- Patient Exit Survey - December 10-14
Please let me know if
- New staff members will be taking the Health Survey so they can be consented
- You'd like a collection box to be sent with the Patient Exit surveys
- Doing surveys between Dec 8 - 12 will present a problem (staff will be out of town, etc.)
Action Plans:
Now is the time to think about how your clinic will roll out AIM-HI with patients on January 1, 2008.
- What methods are being used to train clinicians and staff about the AIM-HI program?
- What changes (posters, videos in the waiting rooms, information in exam rooms) can be made to the clinic to encourage utilization of AIM-HI?
- Where will the 30 study patients complete their step tests and meet with study coordinators?
- Who will meet with the 30 study patients, acting as study coordinator?
- How will AIM-HI tools (health inventory forms, RX pads, study manuals, etc.) but used.

Additional Information:
- Spanish AIM-HI buttons are being shipped - they will arrive directly at Enhanced Offices and will be delivered by Laura at Traditional Offices.

AIM-HI Team Member Update Sep 24, 2007 & Info on Patient Exit Surveys
First and foremost, you all have been doing an excellent job of meeting survey deadlines and the expectations of the study! I cannot thank you all enough because you have done a beautiful job and made my work really easy!
At this point in the study our two enhanced offices (TCC Pine and TCC Main) have attended training in Dallas and are working on roll-out with their staff members. Our traditional offices (FHCCGLA Hawaiian Gardens, FHCCGLA Bell Gardens and CCHC Highland Park) are getting ready to attend their Dallas training session at the end of October. You have all completed a number of surveys, and as I mentioned, have done a great job - I was told that the LA Net clinics had some of the best return rates on their surveys and that is a major accomplishment for a study of this size!
We're about to start a new phase of the study as up until now we've only done clinic staff data collection. Now we'll begin to do surveys with patients. The Patient Exit Survey will start with data collection on Monday Oct. 1st. I've included the instructions from the American Academy of Family Physicians National Resource Network (AAFP NRN) below, please read through these and let me know if you have any questions, concerns, etc.
Instructions for Implementing AIM-HI Patient Exit Survey
The purpose of the Patient Exit Survey is to evaluate the extent to which the AIM-HI program has been implemented in your practice. Sampling of ALL practice patients (not only those who have been enrolled in the research evaluation study) at periodic intervals will give project staff an indication of the program’s “reach” – that is, the extent to which patients in your practice have experienced the AIM-HI program.
Here is how you will implement the Patient Exit Survey in your practice:
1 – We are sending to you sequentially numbered sealed envelopes. The exact number of envelopes will vary from practice to practice, but will generally be about 200 envelopes. In each envelope will be either a Patient Evaluation Survey or a Patient Satisfaction Survey – only the patient will know which survey they are receiving. The purpose of the Evaluation Survey is to evaluate the extent to which the AIM-HI program has been implemented in your practice. The Satisfaction Survey is a general survey regarding how satisfied the patient is with the care received at your practice. Some patients will receive the Evaluation Survey and some the Satisfaction Survey so that practice staff will not know which patients are receiving the Evaluation Survey and consequently will be less likely to change their behavior due to the knowledge that the patient will be completing a survey on AIM-HI.
2 – If you see Spanish-speaking patients, we are sending to you two groups of sequentially-numbered envelopes – one group for Spanish-speaking patients and one group for English-speaking patients. The envelopes for Spanish-speaking patients are numbered 1S, 2S, 3S, etc. Patients should complete the survey in whichever language they are most comfortable. Instructions are included with the survey.
3 – Beginning on MONDAY MORNING, OCTOBER 1st, Front Desk staff will hand EVERY patient who is at least 18 years old and who can read English or Spanish the next sequentially-numbered envelope in English or in Spanish as they check in for their appointment. If English is the primary language, please hand them the next numbered English envelope. If Spanish is the primary language, please hand them the next numbered Spanish envelope. Please keep the pile of English envelopes separate from the Spanish envelopes so that it is easy to hand the patient the next appropriate envelope. The patient should be invited to complete the anonymous survey after they see the doctor, but before they leave the office. It should take them no more than 5 minutes to complete the survey. Each survey is one page – instructions on one side and the survey on the other side.
Please let them know that the survey is completely anonymous and no one in the practice will see the results of the survey. The patient should place the completed survey in the envelope provided, seal the envelope, and return the envelope to the Front Desk staff or to any nurse in the office. If necessary, the patient can complete the survey at home and return it in the postage-paid envelope. However, please don’t encourage this, as frequently patients forget to return surveys that they take home with them. Please give a survey to EVERY patient seen on Monday through Friday during the week, for ALL types of office visits, who is at least 18 years old and reads English or Spanish. Please be enthusiastic about and emphasize the importance of the survey. Your patients will decide to complete or not complete the survey in large part based on the enthusiasm of your staff.
If you run out of surveys before the end of the week, you do not need to hand out any additional surveys.
If you still have remaining surveys which you have not handed out by the end of Friday, please continue the process of handing them out through the following week. However, please do not pass out surveys for more than two weeks. If you have surveys left at the end of two weeks, please return the remaining surveys to the AAFP NRN office.

AIM-HI Team Member Update Aug 10, 2007 - Enhanced Offices
It's almost time for the AIM-HI Enhance Clinic Team Leaders to attend the Dallas training on Aug 24. Before we leave I'd like everyone (Medical Directors, Clinic Managers, Team Leaders attending training and LA Net staff) to speak on a conference call - this will give us a chance to introduce ourselves, address any questions and schedule training times for clinics when the Team Leaders get back to California.
The AAFP has asked Enhanced Clinics to decide when they'll have clinic-wide trainings following the Dallas training because they'd like to schedule a call-in from an AAFP staff member to welcome the clinic to the project. Please discuss when you'd like to hold the clinic-wide training sessions. The amount of time for training staff will likely be 2-3 hours which can be split into segments.
The Enhanced Clinic Team Leader Conference Call will be held on THURSDAY August 16 at 5pm.
AIM-HI Team Member Update Aug 1, 2007
First off, I would like to thank everyone for working so hard to get the most recent surveys completed on time - we had an excellent response rate and it was great getting a chance to meet with so many of the clinic's staff members!
The American Academy of Family Physicians will be providing free T-shirts for AIM-HI lead team members (the people who will join us at the trainings) so please let me know sizes to order by tomorrow (THURSDAY) morning.
Since training for the Enhanced Offices is coming up so quickly, we'd like to start planning training sessions for when we return. The amount of time for training staff will likely be 2-3 hours which can be split into segments. Setting training time now is very important because Healthy Office initiatives need to be rolled out in early September and additional staff health surveys will need to be completed at that time.
We also need to know the approximate number of patients that your practice sees per week and full contact information for lead team members (name, email address and phone).
So to sum it up:
